Therapy Rates

  • $275/ 1Individual session  

  • $ 300/ 50 min Couples Session

EMDR Intensive Rates

  • $525 / 90 minute intake 

  • $1,050/ 3 hour intensive session

  • $175 / 30 minute follow up

Insurance:

Please check with your insurance plan on your out of network benefits for possible reimbursement from your insurance plan. We are happy to provide you with a “superbill” for submitting out of network claims to your insurance plan. Please inform your provider that you need a “superbill” prior to your appointment. 

Payment

We accept all major credit cards & FSA as forms of payment. Payment is due in full at time of service.

No Show/ Late Cancellation Policy – Routine Sessions

If you are unable to attend a session, please make sure you cancel at least 48 business hours beforehand to avoid late cancelation or no-show fees.

Cancellation Policy – EMDR Intensive Sessions

If you are unable to attend a session, please make sure you reschedule within a week beforehand to avoid the loss of your deposit that can be used towards a rescheduled Intensive. The deposit is a non-refundable fee of 50% of your scheduled session.

IMPORTANT INFORMATION

    1    Cancellation/ No show policy: If an appointment is not cancelled at least 48 business hours in advance you will be charged the full rate. Your card will be charged within the 48 business hr window.

    2    Scheduled appointments: If a client is 15 minutes past their scheduled time we will have to reschedule the appointment. The only exception is when you have contacted me and I agree to wait beyond the 15 minutes. You will not be extended any additional time beyond your originally scheduled appointment time.

    3    Account balances: I require that clients with self-pay balances do pay their account balances to zero(0) prior to receiving further services by my practice Clients who have questions about their bills should contact my office at 631-743-6558.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost for self-pay services. At the present time Good Faith Estimates are not required for services covered by insurance.

Under the “No Surprises Act” law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided.

You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services upon request or when scheduling such items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

If you schedule a health care item or service at least 3 business days in advance, make sure your health care provider or facility gives you a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule a health care item or service at least 10 business days in advance, make sure your health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after scheduling. You can also ask any health care provider or facility for a Good Faith Estimate before you schedule an item or service. If you do, make sure the health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after you ask.

If you receive a bill that is at least $400 more for any provider or facility than your Good Faith Estimate from that provider or facility, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate and the bill.
